How to Safely Use an Airway Nasopharyngeal in Emergency Situations
Unfortunately, there will likely come a time when you or someone in your group, is involved in a situation that requires emergency medical attention. It's critical to be prepared for these situations, and one vital skill is the ability to use an airway nasopharyngeal (NPA) safely. The NPA is a device used to maintain an open airway in an unconscious or semi-conscious person. In this article, we will provide a detailed overview of the nasopharyngeal airway, its indications and contraindications, and guidance on how to safely insert the device.

Overview
The primary purpose of using an airway nasopharyngeal (NPA) is to establish and maintain a clear passage through the upper airway in an emergency. Specifically, the device is designed to ensure adequate nasopharynx oxygenation and ventilation in patients at risk of airway obstruction or respiratory compromise. This device is commonly used in emergency and critical care settings to prevent the collapse of the tongue or soft tissues at the back of the throat, allowing unobstructed airflow into the lungs.
Indications and Contraindications for NPA Use
Before using an NPA, it's critical to assess the situation and determine if the device is required. Indications for NPA use include patients with reduced consciousness levels or those who have an altered state of consciousness, including those who have suffered from a head injury, consumed drugs or alcohol, or experienced cardiac arrest. However, some people may not be able to tolerate an NPA, making it critical to be aware of the contraindications. These include, but are not limited to, patients with facial trauma, coagulopathy, or nasal obstruction.
Proper Insertion Technique
Proper technique is crucial when using an NPA to ensure it's inserted safely and effectively. When inserting an NPA, one must follow, at minimum, the following three steps:
- Assess the patient to determine the appropriate size NPA. The ideal NPA should be long enough to reach the patient's nasopharynx and short enough not to cause trauma.
- Lubricate the NPA to ensure comfortable insertion for the patient.
- Insert the NPA through the patient's nostril, ensuring it follows the nasal floor towards the back of the pharynx.
Challenges and How to Address Them
Inserting an NPA can be challenging for many reasons, such as patient anatomy, the presence of blood or secretions, or patient agitation. Always ensure the patient's airway is sufficiently clear before insertion to address these challenges. Suction or clear the airway as needed to remove obstructions and secretions. In cases of patient agitation, it may be necessary to administer sedatives to control the situation.
Continuous Monitoring:
Once the NPA is in place, it must be continuously monitored to ensure it remains in place and the patient's airway is not at risk of obstruction. It's essential to periodically check the NPA's position to ensure it has not moved and that there are no signs of obstruction, such as snoring, choking, or gagging. The color of the patient's lips and nails should also be monitored to ensure proper oxygen flow.
